It was also to prevent accidental dropping of items by clicking on the wrong
item (selling a bunch of stuff in a shop, you don't want to accidentally drop
that artifact.)
However, it would not be hard to extend the client so that something like
control right click sends and unlock and then drop for an item.
Actually, be able to bind the mouse buttons could be a very nice extension.
However, some of it is context sensitive (drop in the inventory is a pickup if
on the ground). So you would really have a binding for the inventory window
and a binding for the look window.
